Lessons Learned from Accidents Involving Chemical Reactions during Maintenance and Cleaning Operations
This report presents the results of the analysis performed on a selection of accidents reported to the MARS database. The study is focused on those accidents that occurred during maintenance or cleaning procedures, involving chemical reactivity hazards. The objective is to obtain lessons learned by studying the causes and consequences of such accidents, so that similar situations can be avoided in the future. Safety management systems should incorporate the lessons learned in order to improve the overall safety of the procedures of routine operations to be performed in an establishment.
